The drum extension allow access to both ends of the cable. You can have a short amount of cable on the extension while the rest is on the reel, which allows you to plug in both ends without having to remove all the cable from the reel. Without it you would have to pull all the cable off the reel to access both ends, since one end would be buried under the rest of the cable on the spool.
